Grainy/slightly blurry recordings

I tried every setting change I could find but i still get grainy/ blurry video.
I tried rate control cqp set anywhere from 13 - 20 and cbr set between 6000-40000.
Different priority settings, look ahead and psycho visual tuning checked and unchecked.
The analyzer through obs doesn't pick up any issues either. (screen shot below heres link as well https://obsproject.com/logs/k3b4OiY8u8OlCOlg)
