Black screen on OBS when plugging in 4k TV in capture card

Please help. I have an ultrasound machine outputting 1080p into a Elgato hd s + into a laptop running OBS. All works fine on a 1080 canvas (can see video feed) until I plug in a 4K TV into the output on the capture card.... Then the video on OBS Studio goes to a black screen but the image is visible on the TV. Any suggestions???

Sorry forgot to mention that it all works fine with Elgato capture software but not OBS. So either it is a computer or OBS setting???
